Jacob (not his real name) had a huge swollen leg and we called it "Elephantiasis"
I cannot recall what symptoms brought him into our ward but he was a quiet unassuming man hardly ever complained but we knew he was in pain and discomfort.
We tried to induce better renal function with medications and even what was called "pyretotherapy". I doubt if it is used much now but his water/renal function would not respond to any treatment. Considering that removing the leg may make him disabled but his general condition may improve and kidney function may get better but the surgeons would not agree. I wanted to send him to another surgeon but Jacob refused saying," I do not wish to see any doctors who do not respect your judgement" I did not realize till then how much respect and regard he had for me
He, like us (doctors) gave up on treatment and just let it go in the hands of the Almighty or the disease (renal failure) to take its normal course. He was not old but still in mid-thirties. Doctors and Nurses all looked after him and weeks passed his condition worsened.
In the mean time another man with similar name (Jacob ii) got admitted with lot of abdominal symptoms (usually described by him in more dramatic terms) that we were trying to investigate. He became sympathetic to Jacob and we put him next to his bed. He would tell us more about Jacob's problems than his own. Anyway in the long run the day rapidly approached when Jacob lapsed into coma and we knew he was dying. His next bed Jacob ii spent his own money for this Jacob's needs as he was well to do. The nurse called me to check him at the time of Jacob's death and the Jacob-ii was standing there. I examined his heart and lungs etc. and declared him dead and also told J-ii to go to his own bed but he said to me in typical dramatic fashion "you came too late doctor". Jacob ii spent from his pocket all the expenses for transporting his body to his relatives who lived far away and could not come.
